    Trauma consultations in a Swiss tertiary emergency department: Comparison of asylum seekers and the local population—Patient characteristics and patterns of injuries, a retrospective study

    BACKGROUND: In 2017, accidents and other acts of violence were the fifth most common cause of death in Switzerland. Moreover, there are increasing numbers of refugees and asylum seekers (AS), who often exhibit distinct disease profiles from those of the natives of the host country. If these differences could be clearly identified, this might help to develop and implement strategies to prevent injuries in health care programs for refugees and asylum seekers. The aim of this study was to examine the types and characteristics of physical trauma profiles in patients from the two largest groups of AS in Switzerland–from Eastern Africa (EA) and the Middle East (ME)–who consulted a Swiss Emergency Department (ED) in 2017/2018. Furthermore, the physical trauma profiles of Swiss national (SN) patients were examined in order to explore potential differences. METHODS: Descriptive retrospective study of adult trauma patients consulting the ED of a Swiss University Hospital between 01/2017 and 12/2018. The study included 157 asylum seeking trauma patients from EA and ME were included in the study. These were matched by gender and age to 157 Swiss trauma patients consulting the ED in the study period. RESULTS: There were significant differences between the groups with respect to type of admission, level of severity, localization and mechanisms of injury. While SN had higher levels of injuries related to road traffic or work, AS had higher levels of injuries related to attempted suicide or to assault. CONCLUSIONS: There were differences between AS and the local population with respect to the characteristics and patterns of injury, so that strategies for preventing injuries and promoting health must be tailored to the target population. Moreover, the observed high rates of outpatient treatment for both groups underline the increasing role of EDs as primary care providers for the population served

    Since December 2019, the world is potentially facing one of the most difficult infectious situations of the last decades. COVID-19 epidemic warrants consideration as a mass casualty incident (MCI) of the highest nature. An optimal MCI/disaster management should consider all four phases of the so-called disaster cycle: mitigation, planning, response, and recovery. COVID-19 outbreak has demonstrated the worldwide unpreparedness to face a global MCI.This present paper thus represents a call for action to solicitate governments and the Global Community to actively start effective plans to promote and improve MCI management preparedness in general, and with an obvious current focus on COVID-19

    Legal linked data ecosystems and the rule of law

    This chapter introduces the notions of meta-rule of law and socio-legal ecosystems to both foster and regulate linked democracy. It explores the way of stimulating innovative regulations and building a regulatory quadrant for the rule of law. The chapter summarises briefly (i) the notions of responsive, better and smart regulation; (ii) requirements for legal interchange languages (legal interoperability); (iii) and cognitive ecology approaches. It shows how the protections of the substantive rule of law can be embedded into the semantic languages of the web of data and reflects on the conditions that make possible their enactment and implementation as a socio-legal ecosystem. The chapter suggests in the end a reusable multi-levelled meta-model and four notions of legal validity: positive, composite, formal, and ecological

    On January 2020, the WHO Director General declared that the outbreak constitutes a Public Health Emergency of International Concern. The world has faced a worldwide spread crisis and is still dealing with it. The present paper represents a white paper concerning the tough lessons we have learned from the COVID-19 pandemic.     Diversity and ethics in trauma and acute care surgery teams: results from an international survey

    Background Investigating the context of trauma and acute care surgery, the article aims at understanding the factors that can enhance some ethical aspects, namely the importance of patient consent, the perceptiveness of the ethical role of the trauma leader, and the perceived importance of ethics as an educational subject. Methods The article employs an international questionnaire promoted by the World Society of Emergency Surgery. Results Through the analysis of 402 fully filled questionnaires by surgeons from 72 different countries, the three main ethical topics are investigated through the lens of gender, membership of an academic or non-academic institution, an official trauma team, and a diverse group. In general terms, results highlight greater attention paid by surgeons belonging to academic institutions, official trauma teams, and diverse groups. Conclusions Our results underline that some organizational factors (e.g., the fact that the team belongs to a university context or is more diverse) might lead to the development of a higher sensibility on ethical matters. Embracing cultural diversity forces trauma teams to deal with different mindsets. Organizations should, therefore, consider those elements in defining their organizational procedures. Level of evidence Trauma and acute care teams work under tremendous pressure and complex circumstances, with their members needing to make ethical decisions quickly. The international survey allowed to shed light on how team assembly decisions might represent an opportunity to coordinate team member actions and increase performance

    International audienceTechnological improvements offer new forms of communication and self-expression. Their use can sometimes leads to undesirable statements, exclusions and isolations, human rights violations and undemocratic practices. In recent years, digital storytelling methodology has become increasingly popular as a multimedia storytelling educational process, both creative and educational, that encourages critical thinking and democratic behavior. Starting from a research-creation project in the framework of an IDEFI-CreaTIC workshop dedicated to the Odyssey, editorialized and remediated through the collections of the French National Library in collaboration with the University of Athens, we will expose in a critical way the question of digital storytelling and we will advance the hypothesis that this methodology, both participative and innovative, has a dimension of democratic education that touches the mutations of freedom expression in the digital age.Les avancées technologiques proposent des formes inédites de communication et d’expression de soi. Leur usage peut parfois conduire à des énoncés indésirables, à des exclusions et des isolations, des violations de droits de l'homme et des pratiques non démocratiques. Ces dernières années, la méthodologie du digital storytelling devient de plus en plus populaire en tant que procédé pédagogique de narration multimédia, à la fois créatif et éducatif, qui encourage la réflexion critique et le comportement démocratique. Ayant comme point de départ un projet de recherche-création dans le cadre d’un atelier-laboratoire IDEFI-CréaTIC consacré à l’Odyssée, éditorialisée et remédiatisée à travers les collections de la BnF en collaboration avec l’Université d’Athènes, nous exposerons d’une manière critique la question du digital storytelling et nous avancerons l’hypothèse que cette méthodologie, à la fois participative et innovante, revêt une dimension d’éducation démocratique qui touche les mutations de la liberté d'expression à l'ère numériqu
